Safety forms app for Advance Excavations

Site supervisors fill in and sign Safe Work Method Statements on an iPad. The finished PDF files itself into the right project folder.

The problem

  • Every job needs a Safe Work Method Statement (SWMS) filled in, signed off by the supervisor and signed on by the crew.
  • Their statements were set Word layouts, and supervisors work from iPads out on site.
  • The signed copy then had to end up in the right project folder so it could be found later.

What we built

  • A web app with Microsoft sign-in, so staff use the account they already have.
  • 35 SWMS templates as tiles. Pick the job type, then the client and location, which come straight from their SharePoint contract folders.
  • PPE, risk rating (likelihood by consequence), site hazards, supervisor signature and a crew sign-on sheet.
  • Works offline. Forms are held on the iPad and sent when signal comes back.
  • On submit, a PDF in their own layout is made and filed into SharePoint under the project, with no one filing it by hand.
  • A monthly report goes to the office at 6am on the first business day, with charts and an Excel file.
  • Hosted on a server in Perth.

The outcome

  • Live since June 2026.
  • By late September, 27 real forms had been submitted and all 27 PDFs were made and filed, with no failed jobs.
  • 26 clients and 145 work locations synced in from SharePoint, so nobody types them in.
  • 10 different SWMS types in use on real jobs.
  • The monthly report has run since October 2026.
